I created a email dorm and choose options below

Hmm....
Go To CP>Email Forms>Mail Forms Management>Select Your Form and Click Field Constructor. Now Modify Your Form>Go Back To Mail Forms Management>Rebuild Forum Template > Done!! lokbatanli, we have checked your website. You didn't press "Rebuild form template". So I suggest you go CP->Main » E-mail Forms » Mail forms management->Field constructor->make all the necessary changes and Save them. But after that go back to Mail forms management and press Rebuild form template. That has to do it.
