1. Do I place the all meta tags together in between <head> and </head>?

2.Do I place the meta tags on all pages displayed in page editor> page management?(In the pages I have created, I don't see a <head> tag in the html code anywhere)

3.Do i place the meta tags in all the module pages as well? Like the module main page, page with section comments, page with category comments, appearance of entries etc of ad board and other modules too?

4. What is <?$META_DESCRIPTION$?>, do I remove it or what?
