I created a email dorm and choose options below

Hmm....

Go To CP>Email Forms>Mail Forms Management>Select Your Form and Click Field Constructor. Now Modify Your Form>Go Back To Mail Forms Management>Rebuild Forum Template > Done!!

After Appearing The Template, Just Close That. Your Form Is Modified Now!!

lokbatanli, we have checked your website. You didn't press "Rebuild form template". So I suggest you go CP->Main » E-mail Forms » Mail forms management->Field constructor->make all the necessary changes and Save them. But after that go back to Mail forms management and press Rebuild form template. That has to do it.
Please, use search next time and continue in the provided thread.
