Is it possible to have Custom Email Form

by means when guest or user adds entry they have their own email form through which they can be contacted on their email

Yes, It is possible to have a custom email form. In order to create a custom email form. Go to CP -> Email Forms -> Mail Forms Management -> Find the Form you want and edit it.

And No, It is not possible to customize the email forms based on the members.

NormalX, email forms can be only created through the Control Panel, and the max. number is 50. You can add a field to the new entry form for email addresses.
